  • Facilitate timely access to home health, DME, hospice, custodial care, and other community resources.

  • Provide orders for portable blood draws, xrays, ultrasounds, EKGs, ECHOs, etc.

  • Improve and preserve functional status.

  • Serve as an educator and mentor for patients, families, peers, and other health care personnel.

  • Effective patient advocacy in navigating a complex health care system.

  • Collaborate with specialists.

THREE-IN-ONE

PRIMARY CARE
SERVICES

For patients who are frail, homebound, or require considerable effort to leave the home to transport to the primary care office. There is no additional costs from a regular doctor visit, co-pays do apply.

TRANSITIONAL CARE SERVICES

For patients with a high risk of being rehospitalized due to chronic illnesses and who have recently been discharged home from the hospital. This is a 30-day program to add another layer of support for ensuring a successful transition to the patient’s home environment and prevent rehospitalizations.

PALLIATIVE CARE SERVICES

For patients facing a serious illness and/or multiple chronic illnesses, seeking quality and comfort care over curative or aggressive treatment, and requiring advanced care planning and access to resources. Palliative care strives to provide effective collaborative health care consultation to manage pain and symptoms, and mitigate exacerbations and crises.
